What if your cells had their own cleanup crew?

This week, Dr. Stephen Barrett sits down with Leslie Kenny—CEO of Oxford Healthspan and co-founder of the Oxford Longevity Project—to explore the science of spermidine, autophagy, and healthy aging.

At just 39, Leslie was diagnosed with both rheumatoid arthritis and lupus during her fifth round of IVF. Refusing to accept a bleak prognosis, she embarked on a journey through immunology, longevity research, and lifestyle medicine that ultimately led her to one of the most fascinating compounds in aging science: spermidine. (Can Spermidine Slow Aging)

🔥 In this episode:

  • What autophagy is and why it declines with age
  • The Nobel Prize–winning science behind cellular recycling
  • Senescent “zombie” cells, SASP, and chronic inflammation
  • Why spermidine is considered a senomorphic rather than a senolytic
  • Natural food sources of spermidine and the story behind Primadine

Show Notes from this episode

Episode Overview

At 39, in the middle of her fifth round of IVF, Leslie Kenny was diagnosed with rheumatoid arthritis and lupus and told to expect a steady decline. Guided by her grandmother’s advice to “throw the kitchen sink at it,” she dove into the research on immune modulation, inflammaging, diet, stress and trauma, and describes how her lab markers normalized and she has remained in remission for 21 years.

That personal journey led Leslie to Oxford, where she encountered research on spermidine, a naturally occurring polyamine that supports autophagy, the cell’s self-cleaning and recycling system. Dr. Barrett and Leslie unpack what autophagy is, how it declines with age, the difference between senolytics and senomorphics, and why zombie cells, SASP and cardiomyocyte maintenance matter. Along the way they detour into Dr. Kevin Tracey’s vagus nerve work, the ACE test, intergenerational stress, and thyroid physiology, then close with how Leslie sources Primadine from Japanese wheat germ and Okinawan chlorella.

Show Notes

00:00         Cold Open: Meet Leslie Kenny. Stephen introduces Leslie Kenny, CEO of Oxford Healthspan and co-founder of the Oxford Longevity Project, previews the autophagy and spermidine conversation, and shares the listener code POI15 for 15% off Primadine.

01:32         Welcome and Setting the Stage. Stephen welcomes Leslie and explains why he wants her origin story on the table before the deep dive into spermidine.

02:12         The Diagnosis: Rheumatoid Arthritis and Lupus at 39. In 2004, in the middle of her fifth round of IVF, hand pain leads to diagnoses of RA and lupus, a prognosis of steady decline, and a sobering piece of advice from her physician.

07:12         “Throw the Kitchen Sink at It”. Guided by the voice of her late grandmother, Leslie resolves to become the best possible patient, aiming to be just 1% better each day, and turns to patient bulletin boards for answers.

08:50         The IVIG Study and Immune Modulation vs. Suppression. A small German study of 13 lupus patients treated with intravenous immunoglobulin catches her eye. She explains the yin-yang, balance-of-the-body philosophy behind her reluctance to simply suppress her immune system.

11:25         When Evidence Isn’t “Statistically Significant”. Her own physician declines to prescribe IVIG, so Leslie seeks out a San Francisco doctor willing to treat her, leading to two IVIG rounds in Denver.

12:19         Inflammaging and the Lifestyle Reset. Claudio Franceschi’s concept of “inflammaging,” allergy testing that flagged gluten, dairy and eggs, going lectin- and nightshade-free, cutting stress, moving more, and pursuing trauma therapy.

15:00         Labs Return to Normal. Leslie recounts a checkup within six months of diagnosis when her CRP, IL-6 and TNF-alpha had normalized and her doctor told her the lupus and RA markers were gone.

16:20         A System That Doesn’t Reward Curiosity. Why her doctor didn’t ask what she had changed, the time pressures facing physicians, and the case for patients and doctors meeting each other halfway.

17:55         Motherhood and a Fateful Playground Conversation. Adoption, a surprise pregnancy at 43, and the Oxford school-playground chats that introduce her to researchers working on transcranial stimulation, stem cells and regenerative medicine.

19:45         Sponsor Message: ValAsta Astaxanthin. Stephen shares his experience with ValAsta astaxanthin for inflammation, gut health and recovery. Use code POI5 at valasta.net for 5% off.

20:36         From Oxford Spinouts to Spermidine. Leslie’s Harvard MBA background leads her to help Oxford regenerative medicine spinouts raise capital, and to rheumatology researchers whose work showed spermidine improved vaccine responses in older mice.

22:32         Dr. Kevin Tracey and Vagus Nerve Stimulation for RA. Stephen recounts his earlier interview with Dr. Kevin Tracey, the FDA indication for vagus nerve stimulation in rheumatoid arthritis, and how the approach resets immune “set points” rather than blocking a single pathway.

25:48         Vagal Tone, Trauma and Chronic Low-Grade Inflammation. Leslie connects parasympathetic balance to unresolved stress and references Bessel van der Kolk’s The Body Keeps the Score.

26:35         The ACE Test Explained. What the Adverse Childhood Experiences (ACE) score measures, why it correlates with poorer health outcomes, and why patients who feel they “got over it” may want to take a second look.

29:01         Ceremony, Closure and Many Roads to Remission. Letter writing and ritual as trauma therapy, plus Leslie’s view that holistic care, IVIG, vagal stimulation and hormone therapy are different roads patients have used to calm autoimmunity.

30:43         Treating the Core. Stephen on targeting root causes (type 2 diabetes as the example), the link between childhood trauma and chronic pain, and rodent studies showing stress carried across generations.

33:10         Famine, Intergenerational Trauma and the Thyroid. Outcomes among offspring of the Dutch Hunger Winter, descendants of the Irish famine and hypothyroidism as a protective slowdown, and how T4-to-T3 conversion works like a thermostat.

35:06         Hashimoto’s, Remission and a Fertility Surprise. How an ultrasound revealed Hashimoto’s thyroiditis, why Leslie credits her broader approach for 21 years of remission, and the unexpected result once her thyroid was treated.

37:29         What Is Autophagy?. “Self-eating” reframed as “self-cleaning”: how cells recycle dysfunctional mitochondria and proteins, and why Leslie calls autophagy the Marie Kondo of the cell.

38:39         Sponsor Message: BBack 0524. Stephen shares his first run in the BBack 0524, a shoe classified as a medical device in Europe. Use code POI10 at bbackworld.us for 10% off.

39:31         Autophagic Flux and the 2016 Nobel Prize. Why cellular renewal declines with age, and how Yoshinori Ohsumi’s yeast experiments earned the 2016 Nobel Prize in Physiology or Medicine.

40:46         Senescence, Zombie Cells and SASP. The “Uncle Joe at the wedding” analogy for senescent cells and their inflammatory SASP signals, why cardiomyocytes need maintenance like vintage cars, and why Leslie describes spermidine as a senomorphic that helps cells “sober up” rather than a senolytic that kills them.

45:23         Losing Our Parts: Spermidine, Testosterone, Melatonin and Nitric Oxide. Stephen and Leslie on the many molecules that decline with age, and why she thinks of herself as a 1965 Mustang.

45:55         Where Spermidine Comes From. Natural production begins to decline in our late 20s, and spermidine is found in high concentrations in breast milk to support rapid growth.

46:21         Oxford Healthspan and the Origin of “Primadine”. Synthetic versus food-derived spermidine, and how an 87-year-old mother inspired the name Primadine, for “the new prime of life.”

47:55         Dietary Sources and the Blue Zones. Gut bacteria, shiitake and oyster mushrooms, legumes, sprouts, fermented cheese and wheat germ, and why plant-rich blue zone diets may matter.

49:30         Sourcing Clean: Japan, Wheat Germ and Okinawan Chlorella. Herbicide concerns in the food supply, Japan’s strict regulations, wheat germ spermidine with fragile omega-6 fats removed, and an Okinawan chlorella strain grown with turmeric and lime peel (nobiletin).

51:55         Powders, Rituals and Recipes. Sprinkling spermidine into a matcha latte, yogurt, smoothies or oatmeal, and even baking it into a key lime pie.

52:42         A Gluten-Free Option for the Autoimmune Community. Why Leslie developed a chlorella-based version after listeners on autoimmune paleo diets asked, with no fillers, binders or flow agents.

53:30         Where to Learn More and Get Your Discount. Code POI15 at oxfordhealthspan.com, spermidine’s reported effect on nine of the twelve hallmarks of aging, free Oxford Longevity Project videos from Rhodes House conferences, and where to follow Leslie’s story.

55:37         Closing Thoughts. Stephen reflects on patient empowerment and finding alternate paths when mainstream care runs short on time, plus a shout-out to fellow Hoosiers.

56:31         Sponsor Message and Outro: Approved Medical Solutions. An oxalate-free nitric oxide formula from Approved Medical Solutions, Stephen’s call to leave a review on Apple Podcasts, and the standard disclaimer (59:01).
