Is insulin resistance a bigger driver of heart disease than cholesterol? Cardiothoracic surgeon Dr. Philip Ovadia joins The Pod of Inquiry to challenge some of the most deeply held assumptions in modern cardiology.
Dr. Ovadia knows the problem firsthand. Despite being a practicing heart surgeon, he became morbidly obese and pre-diabetic while following the conventional health advice he had been taught. After losing 100 pounds and reversing his pre-diabetes, he began questioning whether cardiology has focused too heavily on cholesterol while overlooking metabolic health. (The Real Cause of Heart Disease)
In This Episode we explore:
- Insulin resistance vs. LDL cholesterol — which better predicts cardiovascular risk?
- Why Dr. Ovadia says metabolic health may be a critical missing piece in heart disease prevention
- The Women’s Health Initiative findings he believes deserve more attention
- His own experience with very high LDL levels and a coronary calcium score of zero
- What the Keto CTA trial suggests about LDL, ApoB, and actual coronary plaque progression
- Small, dense vs. large, buoyant LDL particles and why particle characteristics may matter
- The relationship between insulin resistance, metabolic health, and cardiovascular disease
- Dr. Ovadia’s criticism of conventional cardiology and the American Heart Association
- The role of gut health, nutrition, and animal foods in his broader metabolic-health approac
- Insights from his new book, Stay Off My Kitchen Table
If you’ve ever wondered whether cholesterol is the whole story behind heart disease, this conversation offers a provocative look at the metabolic factors that may deserve more attention.

Dr. Philip Ovadia
Dr. Ovadia is a board-certified cardiothoracic surgeon who has performed over 3,000 heart surgeries. After becoming morbidly obese and pre-diabetic during his own surgical training, he reversed his health through a low-carbohydrate, animal-based approach — losing over 100 pounds and inspiring his mission to keep patients off the operating table. He is the founder of Ovadia Heart Health, a nationwide telemedicine practice focused on metabolic health as the primary strategy for preventing and reversing heart disease, and the author of Stay Off My Operating Table and the newly released Stay Off My Kitchen Table.

Show Notes from this episode
00:00 Introduction
Dr. Barrett introduces Dr. Philip Ovadia, a board-certified cardiothoracic and general surgeon running a nationwide telehealth practice, previewing a conversation on the metabolic drivers of cardiovascular disease and how insulin resistance changes the prevention picture.
02:06 A Surgeon Who Became His Own Cautionary Tale
Dr. Ovadia describes becoming morbidly obese and pre-diabetic during his cardiothoracic training, realizing conventional “eat less, move more” advice wasn’t working, and losing 100 pounds and reversing his pre-diabetes once he discovered low-carb metabolic health — the origin story behind Stay Off My Operating Table.
04:49 Five Numbers That Predict a Heart Attack
Ovadia lays out the five metabolic health markers — triglycerides, HDL, fasting glucose/A1c, blood pressure, and waist circumference — and cites data showing only 12% of U.S. adults meet all five, meaning 88% are metabolically unhealthy by at least one measure.
08:06 The Neuropathy Parallel
Dr. Barrett shares his own clinical experience ordering fasting insulin on pre-diabetic peripheral neuropathy patients and finding rampant undiagnosed hyperinsulinemia — a test rarely ordered even by endocrinologists managing the same patients.
10:40 Nattokinase, Thrombosis, and Carotid Plaque
The two discuss a study on nattokinase and reversal of carotid (not coronary) plaque, with Ovadia explaining the compound’s role in reducing thrombosis — a downstream consequence of insulin resistance and inflammation rather than a fix for the root cause.
14:48 Sponsor Break: ValAsta Astaxanthin
Dr. Barrett shares his own experience with ValAsta’s bioavailable astaxanthin for inflammation and recovery. Code POI5 at ValAsta.net for 5% off.
17:34 The Study That Should Have Changed Everything
Ovadia details Women’s Health Initiative data showing elevated LDL raised heart disease risk by 40%, while an abnormal LPIR (lipoprotein insulin resistance) score raised it by 600% — yet cholesterol remains the only number most doctors discuss with patients.
21:30 Living Proof: The Keto CTA Trial
Referencing colleague Nick Norwitz and the Keto CTA trial, Ovadia explains his own eight-year LDL of 250–350 with a coronary calcium score of zero, and how the trial found no correlation between LDL/ApoB levels and one-year plaque progression in a cohort of high-LDL, metabolically healthy participants.
23:54 Sponsor Break: BBack 0524 Footwear
Dr. Barrett reviews the BBack 0524 shoe, a biomechanically supportive design classified as a medical device in Europe. Code POI10 at BBackWorld.us for 10% off.
25:37 One Root Cause, Many Specialties
The conversation turns to how neurology and cardiology give conflicting cholesterol guidance, and how Alzheimer’s, Parkinson’s, and other neurodegenerative conditions share the same insulin-resistance root cause as heart disease — meaning patients needn’t choose between heart risk and cognitive risk.
28:30 Is the American Heart Association Captured?
Ovadia argues the American Heart Association has opposed the 2025 federal dietary guideline reversal (which restored permission to eat red meat, dairy, and animal fat) despite seven decades of failing to reduce heart disease, and questions whether food and pharmaceutical funding shapes its recommendations.
32:13 Why Statins Don’t Work for Everyone
Ovadia distinguishes small, dense (atherogenic) LDL particles from large, buoyant (benign) ones, explains that insulin resistance shifts particle size toward the harmful kind, and cites statin and PCSK9 trial subgroup data showing no benefit for people who are metabolically healthy regardless of LDL level.
37:42 Stay Off My Kitchen Table
Ovadia introduces his new book, which moves beyond “what to eat” into gut health and nutrient bioavailability — and explains why animal products, including red meat, are essential rather than harmful, contrary to his early surgical training.
40:49 Where to Find Dr. Ovadia
Ovadia points listeners to his nationwide telemedicine practice (iFixHearts.com), licensed in all 50 states plus D.C., and his social handle @iFixHearts.
42:52 Closing / Sponsor: Nature’s Marvels
Dr. Barrett shares his three-year personal experience with Nature’s Marvels bioregulatory peptides as part of his own wellness routine, then closes the show and invites listeners to leave a review.
