When was the last time your doctor asked about your gums? 🦷

Polycystic ovary syndrome (PCOS) affects roughly 1 in 8 women, and an estimated 70% remain undiagnosed. But what if oral health plays a bigger role in hormonal and metabolic health than most people realize?

On this episode of The Pod of Inquiry, Dr. Stephen Barrett welcomes Christine Horton, RDH, a registered dental hygienist and certified health coach who was diagnosed with PCOS at 17. Together, they explore the connection between the oral microbiome, gum disease, systemic inflammation, insulin resistance, and PCOS.

Christine also discusses the proposed shift toward the term polyendocrine metabolic ovarian syndrome (PMOS), emphasizing the broader metabolic and endocrine factors involved in the condition. (Can Gum Health Affect Your Hormones)

In this episode, we explore:

Show Notes

Polycystic ovarian syndrome affects roughly one in eight women, yet an estimated 70% of them don’t know they have it. In this episode, Dr. Stephen Barrett sits down with Christine Horton, a registered dental hygienist and certified health coach who was diagnosed with PCOS at 17, to explore a connection most clinicians never consider: the mouth.

Christine explains why PCOS was renamed polyendocrine metabolic ovarian syndrome (PMOS) in May 2026, what is actually happening in the ovary, and why insulin resistance and systemic inflammation sit at the heart of the condition. Then the conversation turns to oral-systemic health: how gum disease and systemic inflammation feed each other, why treating periodontitis can lower A1c, and how antiseptic mouthwash can sabotage nitric oxide production. Along the way, they cover probing depths and periodontal staging, hidden sugars, vaping and nicotine pouches, xylitol, oil pulling, and the root canal controversy.

Episode Timestamps

00:00  Opening. Dr. Barrett introduces Christine Horton, RDH, and the link between the oral microbiome and PCOS.

01:22  PCOS gets a new name. Christine explains the May 2026 rename to polyendocrine metabolic ovarian syndrome (PMOS), and Dr. Barrett compares the name changes to resetting a password.

02:05  Christine’s story. From dental assistant to hygienist, a PCOS diagnosis at 17, an infertility journey, and a perimenopausal crisis that led her to health coaching and oral-systemic health.

05:29  Standard of care, then and now. Why many women still hear the same message decades later; roughly 1 in 8 women have PCOS/PMOS, and an estimated 70% are undiagnosed. Christine’s advocacy work with the PCOS Awareness Association.

07:19  Conventional treatment. Birth control to hold the hormone cascade steady, weight loss advice, and why the new name adds metabolic and endocrine components beyond the ovaries.

09:13  What’s actually happening in the ovary. Not true cysts, but follicles that fail to release, the ultrasound “string of pearls,” and senescent follicles.

10:42  The metabolic thread. 70–80% of women with PCOS/PMOS have insulin resistance. Dr. Barrett draws the parallel to diabetic neuropathy, nephropathy and retinopathy as one metabolic disease in different organs.

13:16  Rotterdam criteria. Why two-of-three diagnostic criteria means two women with the same diagnosis can have very different symptoms.

13:38  Systemic inflammation and risk. Higher rates of cardiovascular disease, stroke, diabetes, depression, anxiety and sleep apnea.

14:54  Breaking the medicine–dentistry silo. Christine’s clinical observations of gum sensitivity and inflammation in women with PCOS.

16:24  Sponsor: ValAsta. Astaxanthin for inflammation and recovery. Code POI5 for 5% off at valasta.net.

17:49  Chicken or egg?. Does the mouth drive systemic inflammation, or vice versa? Both. Treating periodontitis can improve A1c in diabetic patients.

19:23  Mouthwash and nitric oxide. Dr. Barrett recounts Dr. Nathan Bryan’s work showing that antiseptic mouthwash can wipe out the oral bacteria needed for the nitrate–nitrite–nitric oxide pathway, with effects on blood pressure.

21:18  From gums to bloodstream. Inflamed tissue as a doorway for bacteria, the case for saliva testing, and why a daily kill-everything mouthwash is like taking an antibiotic every day.

22:49  A nitrate lunch experiment. Christine’s conference story: nitrate test strips before and after a meal built around beets and pickled foods.

24:03  Nitric oxide and peripheral neuropathy. Why Dr. Barrett tests nitric oxide in neuropathy patients, twice-daily versus once-daily dosing, and how a visible metric drives patient compliance.

25:23  Know your periodontal status. Why anyone with an inflammation-driven disease should ask their dental team about their gums.

27:17  The finger analogy. We would never just amputate an infected finger, so why do we treat teeth as disposable?

28:38  Probing depths, staging and grading. Six points per tooth, 0–3 mm without bleeding as healthy, and how diabetes and tobacco use affect grading.

31:47  Does a healthier mouth help PCOS?. Christine’s experience with patients when oral disease is brought under control.

32:18  Sponsor: BBack 0524. Dr. Barrett’s run without orthotics in the BBack 0524. Code POI10 for 10% off at bbackworld.us.

34:00  Turning gingivitis around. Disrupt, don’t sterilize: the shift away from killing all bacteria, and the hidden sugar in barbecue sauce, soda and iced tea.

36:19  Vaping, pouches and nicotine as a nootropic. Pure nicotine versus tobacco products, dose creep, and what the research does and doesn’t show.

38:58  Sugar substitutes. Dr. Barrett raises COVID-era data on smokers; Christine explains why xylitol is the most-researched sweetener in dentistry, including xylitol sprays for seniors in extended care.

40:38  Oil pulling. 20–30 minutes of swishing, clogged pipes, and why the evidence isn’t strong enough for a blanket recommendation.

42:37  Root canals. The controversy, the root canal–or–extraction decision, and why prevention is the best answer.

46:02  Medicine’s slow boat. Phage therapy, discovered in 1913, and why a residency is only a ticket into lifelong learning.

47:25  Where to find Christine. fusionhealthcoach.com and @PMOSPCOSHygienist on social media.

48:18  Sponsor: Approved Medical Solutions. An oxalate-free nitric oxide formula with no beets, spinach or arginine. Patients: code SBarrett for 10% off; practitioners: register and use Pod of Inquiry at checkout.

Key Takeaways

  • PCOS is now PMOS (polyendocrine metabolic ovarian syndrome), reflecting that it is a metabolic and endocrine condition, not just an ovarian one.
  • Roughly 1 in 8 women have PCOS/PMOS, an estimated 70% are undiagnosed, and 70–80% have insulin resistance.
  • Oral and systemic inflammation run in both directions. Treating periodontitis can measurably improve A1c.
  • Killing all oral bacteria with daily antiseptic mouthwash can disrupt the nitrate-to-nitric-oxide pathway. The goal is to disrupt harmful biofilm, not sterilize the mouth.
  • Ask your dental team for your periodontal stage and grade. Healthy probing depths are 0–3 mm with no bleeding.
  • Prevention beats treatment: every extraction, root canal or implant starts a cascade that is costlier than staying healthy.
